Initial Appointment:

For adult clients: Please print and complete the following forms and bring them with you to your first appointment:

• New Client Information Form
• Professional Services Agreement-Office Copy
• Cancellation Policy

Please read, print, and retain the following forms prior to your initial appointment:

•   HIPAA Policy Form
•   Professional Services Agreement-Client Copy

For children and teens:

• First appointments are with the child's parent(s) and/or legal guardian; the child or teen client does not attend the first appointment.
• If the parents/guardians are divorced or separated, consent for treatment from each parent/guardian will be required prior to scheduling an appointment with the child or teen.

If you are unable to complete the new client forms prior to the first appointment, please arrive 15 to 20 minutes early to complete these forms.

If you are using health insurance, please bring your health insurance card to the first appointment
